What to bring to your appointment

During your first consultation, we will review your symptoms, family medical history, and any previous diagnoses. Diagnostic tests might be conducted during this appointment, or we may refer you for additional testing afterwards.

We will assess your results and discuss with you our advised management and treatment plan. We are committed to providing you with the highest standard of care and helping you live a quality life. 

Please bring to your appointment: 

  • Medicare card (if you have one)
  • Pension / Concession Card (if you have one)
  • Referral letter
  • Any recent pathology results such as blood tests, x-rays, CT scans, 24 hour Holter reports
  • A list of your current medications if not already on your referral letter.

Payment: We accept cash, EFTPOS, or credit card. If Medicare has your bank details, we can process your Medicare rebate refund for you.

Mobility Access: Available via the basement parking levels with the use of the lifts or at the entrance to the building on Fox Valley Road. There are also mobility parking spaces in Parkway San Clinic Car Park.
